Although informed consent is important in clinical research, questions persist regarding when it is necessary, what it requires, and how it should be obtained. The standard view in research ethics is that the function of informed consent is to respect individual autonomy. However, consent processes are multidimensional and serve other ethical functions as well. These functions deserve particular attention when barriers to consent exist. We argue that consent serves seven ethically important and conceptually distinct functions. The first four functions pertain (...) principally to individual participants: (1) providing transparency; (2) allowing control and authorization; (3) promoting concordance with participants' values; and (4) protecting and promoting welfare interests. Three other functions are systemic or policy focused: (5) promoting trust; (6) satisfying regulatory requirements; and (7) promoting integrity in research. Reframing consent around these functions can guide approaches to consent that are context sensitive and that maximize achievable goals. (shrink)

A prefrontal control system that is less mature than the limbic reward system in adolescence is thought to impede self-regulatory abilities, which could contribute to poor dietary choices and obesity. We, therefore, aimed to examine whether structural morphology of the prefrontal cortex and the amygdala are associated with dietary decisions and obesity in children and adolescents. Seventy-one individuals between the ages of 8–22 years participated in this study; each participant completed a computer-based food choice task and a T1- and T2-weighted (...) structural brain scans. Two indices of obesity were assessed, including age- and sex-specific body mass index and waist-to-height ratio. The behavioral task included rating 60 food stimuli for tastiness, healthiness, and liking. Based on each participant’s self-ratings, 100 binary food choices were then made utilizing a computer mouse. Dietary “self-control” was calculated as the proportion of trials where the individual chose the healthier food item over the total number of trials. Cortical thickness and amygdala subnuclei volumes were quantified using FreeSurfer 6.0 and CIT168 atlas, respectively. We found that WHtR was negatively associated with the thickness of bilateral superior frontal, left superior temporal, right insula, and right inferior temporal regions. We also found WHtR to be positively associated with the volume of the central nucleus region of the amygdala, after adjusting for the hemisphere, age, sex, and intracranial volumes. A similar data pattern was observed when BMIz was used. Moreover, we found that across all participants, thinner right superior frontal cortex and larger left CEN volumes predicted lower dietary self-control. These results suggest that differential development of the PFC and amygdala relate to obesity and dietary self-control. Further longitudinal studies are merited to determine causal relationships among altered PFC to amygdala neural circuitry, dietary self-control, and obesity. (shrink)

Infant and young child electrophysiology studies have provided information regarding the maturation of face-encoding neural processes. A limitation of previous research is that very few studies have examined face-encoding processes in children 12–48 months of age, a developmental period characterized by rapid changes in the ability to encode facial information. The present study sought to fill this gap in the literature via a longitudinal study examining the maturation of a primary node in the face-encoding network—the left and right fusiform gyrus. (...) Whole-brain magnetoencephalography data were obtained from 25 infants with typical development at 4–12 months, and with follow-up MEG exams every ∼12 months until 3–4 years old. Children were presented with color images of Face stimuli and visual noise images that served as Non-Face stimuli. Using distributed source modeling, left and right face-sensitive FFG evoked waveforms were obtained from each child at each visit, with face-sensitive activity identified via examining the difference between the Non-Face and Face FFG timecourses. Before 24 months of age the face-sensitive FFG M290 response was the dominant response, observed in the left and right FFG ∼250–450 ms post-stimulus. By 3–4 years old, the left and right face-sensitive FFG response occurred at a latency consistent with a face-sensitive M170 response ∼100–250 ms post-stimulus. Face-sensitive left and right FFG peak latencies decreased as a function of age, and with an adult-like FFG latency observed at 3–4 years old. Study findings thus showed face-sensitive FFG maturational changes across the first 4 years of life. Whereas a face-sensitive M290 response was observed under 2 years of age, by 3–4 years old, an adult-like face-sensitive M170 response was observed bilaterally. Future studies evaluating the maturation of face-sensitive FFG activity in infants at risk for neurodevelopmental disorders are of interest, with the present findings suggesting age-specific face-sensitive neural markers of a priori interest. BackgroundThe increased risk of developing attention-deficit hyperactivity disorder (ADHD) in extremely preterm infants is well-documented. Better understanding of perinatal risk factors, particularly those that are modifiable, can inform prevention efforts.MethodsWe examined data from the Extremely Low Gestational Age Newborns (ELGAN) Study. Participants were screened for ADHD at age 10 with the Child Symptom Inventory-4 (N = 734) and assessed at age 15 with a structured diagnostic interview (MINI-KID) to evaluate for the diagnosis of ADHD (N = 575). We studied associations (...) of pre-pregnancy maternal body mass index (BMI), pregestational and/or gestational diabetes, maternal smoking during pregnancy (MSDP), and hypertensive disorders of pregnancy (HDP) with 10-year and 15-year ADHD outcomes. Relative risks were calculated using Poisson regression models with robust error variance, adjusted for maternal age, maternal educational status, use of food stamps, public insurance status, marital status at birth, and family history of ADHD. We defined ADHD as a positive screen on the CSI-4 at age 10 and/or meeting DSM-5 criteria at age 15 on the MINI-KID. We evaluated the robustness of the associations to broadening or restricting the definition of ADHD. We limited the analysis to individuals with IQ ≥ 70 to decrease confounding by cognitive functioning. We evaluated interactions between maternal BMI and diabetes status. We assessed for mediation of risk increase by alterations in inflammatory or neurotrophic protein levels in the first week of life.ResultsElevated maternal BMI and maternal diabetes were each associated with a 55–65% increase in risk of ADHD, with evidence of both additive and multiplicative interactions between the two exposures. MSDP and HDP were not associated with the risk of ADHD outcomes. There was some evidence for association of ADHD outcomes with high levels of inflammatory proteins or moderate levels of neurotrophic proteins, but there was no evidence that these mediated the risk associated with maternal BMI or diabetes.ConclusionContrary to previous population-based studies, MSDP and HDP did not predict ADHD outcomes in this extremely preterm cohort, but elevated maternal pre-pregnancy BMI, maternal diabetes, and perinatal inflammatory markers were associated with increased risk of ADHD at age 10 and/or 15, with positive interaction between pre-pregnancy BMI and maternal diabetes. W związku z dyskusjami na temat korespondencyjnej, klasycznej teorii prawdy powstały teorie nieklasyczne, łączące pojęcie prawdy z oczywistością, skutecznym działaniem, koherencją, powszechną zgodą itp. Ostra polemika próbuje w ogóle wyeliminować kategorię prawdy. Powstaje pytanie: czy taka radykalna krytyka jest słuszna i uzasadniona? Wydaje się, że jest ona przesadna. Celem artykułu jest argumentacja za tezą, że prawda — jako wartość i norma („wzorzec”, element systemu odniesienia itp.) — jest niezbywalna przy (...) epistemicznej ocenie twierdzeń, które mają swoje konsekwencje teoretyczne i praktyczne. Strukturę artykułu wyznaczają następujące punkty: (1) krytyka kategorii prawdy, (2) prawda jako wartość i (3) prawda jako norma w systemie odniesienia. W punkcie wyjścia przyjmuję syntetyczną charakterystykę prawdy sformułowaną przez Michaela P. Lyncha w jego monografii pt. Prawda i życie. Oprócz tego, bez kategorii prawdy nie można nawet stwierdzić — jak to usiłuje czynić sceptycyzm — że prawdą jest, iż prawda nie jest czymś wartościowym, godnym uwagi itp. Kategoria prawdy — pełniąca funkcję poznawczego celu i normy — pomimo różnych kontrowersji jest wciąż niezbywalna przy ocenie wartości logicznej zdań (i wyrażanych przez nie twierdzeń). Kategoria prawdy jest semantyczna, lecz jej rozpoznawanie — uznawanie zdania przez kogoś za prawdziwe (czyli asercja) — jest kategorią pragmatyczną. Stwierdzana post factum skuteczność działania, podjętego na podstawie jakiegoś zdania, jest mocną racją za tym, aby dane zdanie uznać za prawdziwe. Respekt dla prawdy jest lepszy niż jego brak. This paper discusses what implications global luck egalitarianism (GLE) has for border control. Some authors suggest that an open‐borders policy follows from GLE. The idea is that various unchosen inequalities inevitably follow from differences in birthplace, such that GLE will always have principled reason to condemn closed borders. Others are skeptical of the assumption that GLE will have liberal implications for border control, because open borders may have other, adverse effects that outweigh the reductions in unjust inequality. Against such skeptics, (...) we argue that GLE will (typically) have quite liberal implications for border control. OBJECTIVES: To study the resuscitation preferences, choice of decision-maker, views on the seeking of patients' wishes and determinants of these of elderly hospital in-patients. DESIGN: Questionnaire administered on admission and prior to discharge. SETTING: Two acute geriatric medicine units (Southampton and Poole). PARTICIPANTS: Two hundred and fourteen consecutive consenting mentally competent patients admitted to hospital as emergencies. RESULTS: Resuscitation was wanted by 60%, particularly married and functionally independent patients and those who had not already considered it. Not wanted resuscitation was (...) associated with lack of social contacts. Sixty-seven per cent welcomed enquiry about their preferences and 78% wanted participation in decision, 43% as sole decision-maker. Wishing to choose oneself was associated with not wanting resuscitation, prior knowledge of it, and lack of a spouse. Patients' opinions remained stable during their admission. CONCLUSIONS: Discussion of resuscitation is practical on hospital admission without causing distress and the views expressed endure through the period of hospitalisation. Elderly patients' attitudes depend partly on personal health and social circumstances. This may assist doctors when patients are unable to participate themselves. (shrink)

In the semantic literature, there is a class of examples involving anaphora in intensional contexts, i.e. under the scope of modal operators or propositional attitude predicates, which display anaphoric relations that appear at first glance to violate otherwise well-supported generalizations about operator scope and anaphoric potential. In Section 1,I will illustrate this phenomenon, which, for reasons that should become clear below, I call modal subordination; I will develop a general schema for its identification, and show how it poses problems for (...) most theories of scope and anaphoric relations. In Section 2, I will review the main approaches which have been considered in attempting to account for modal subordination and argue that only an approach involving accommodation can account for the full range of examples. The notion of accommodation is due to Lewis, who defines it as follows: If at time t something is said that requires presupposition P to be acceptable, and if P is not presupposed just before t, then - ceteris paribus and within certain limits - presupposition P comes into existence at t.(Lewis 1979: 340) The interesting question, of course, is what the limits on accommodation might be. I believe that the proper account of modal subordination has something to say about this. I will argue this briefly in section 3, where I draw some conclusions and also sketch some ... (shrink)

Most lymphocytes of the T cell lineage develop along the CD4/CD8 pathway and express antigen receptors on their surfaces consisting of clonotypic αβ chains associated with invariant CD3‐γδε components and ζ chains, collectively referred to as the T cell antigen receptor complex (TCR). Expression of the TCR complex is dynamically regulated during T cell development, with immature CD4+CD8+ thymocytes expressing only 10% of the number of αβ TCR complexes on their surfaces expressed by mature CD4+ and CD8+ T cells. Recent (...) evidence demonstrates that low surface TCR density on CD4+CD8+ thymocytes results from the limited survival of a single TCR component within the ER, the TCRα chain, which has a half life of only 15 minutes in immature thymocytes, compared to >75 minutes in mature T cells. Instability of TCRα proteins in immature CD4+CD8+ thymocytes represents a novel mechanism by which expression of the multisubunit TCR complex is quantitatively regulated during T cell development. In the current review we discuss our recent findings concerning the assembly, intracellular transport, and expression of αβ TCR complexes in CD4+CD8+ thymocytes and comment on the functional significance of TCRα instability during T cell development. BackgroundClinical ethics case consultations (CECCs) provide a structured approach in situations of ethical uncertainty or conflicts. There have been increasing calls in recent years to assess the quality of CECCs by means of empirical research. This study provides detailed data of a descriptive quantitative and qualitative evaluation of a CECC service in a department of cardiology and intensive care at a German university hospital.MethodsSemi-structured document analysis of CECCs was conducted in the period of November 1, 2018, to May 31, 2020. All documents were analysed by two researchers independently.ResultsTwenty-four CECCs were requested within the study period, of which most (n = 22; 92%) had been initiated by physicians of the department. The patients were an average of 79 years old (R: 43–96), and 14 (58%) patients were female. The median length of stay prior to request was 12.5 days (R: 1–65 days). The most frequent diagnoses (several diagnoses possible) were cardiology-related (n = 29), followed by sepsis (n = 11) and cancer (n = 6). Twenty patients lacked decisional capacity. The main reason for a CECC request was uncertainty about the balancing of potential benefit and harm related to the medically indicated treatment (n = 18). Further reasons included differing views regarding the best individual treatment option between health professionals and patients (n = 3) or between different team members (n = 3). Consensus between participants could be reached in 18 (75%) consultations. The implementation of a disease specific treatment intervention was recommended in five cases. Palliative care and limitation of further disease specific interventions was recommended in 12 cases.ConclusionsTo the best of our knowledge, this is the first in-depth evaluation of a CECC service set up for an academic department of cardiology and intensive medical care. Patient characteristics and the issues deliberated during CECC provide a starting point for the development and testing of more tailored clinical ethics support services and research on CECC outcomes. BackgroundResistance training can offer beneficial physiological and psychological effects. The regular continuation of this exercise can be accomplished by improving the recovery and mood after a workout. Frequency-specific microcurrent might offer a solution here as it has been shown to improve physical injuries, mood state, and sleep. However, knowledge is lacking about the impact of microstimulation after RT on said parameters. The present study aimed to test the effects of RT and muscle-microstimulation on mood and physical recovery in healthy men (...) after performing conventional deadlifts, which is a type of RT.MethodsThe study was conducted according to a single-blind, randomized, placebo-controlled, and two-way crossover study. Twenty participants naïve to microstimulation engaged in RT twice on separate days. They were randomized to receive MS on 1 day and no microstimulation on another day. Before and after the workout and after their treatment, participants self-rated their mood state and mental and physical exhaustion levels.ResultsFindings showed that MS increased the self-ratings of well-rested and sociable and, most importantly, reduced the feeling of exercise-induced exhaustion. There were no MS effects on ratings of feeling sad, happy, or exhausted, although the workout, independent of MS, negatively influenced the level of exhaustion.ConclusionThe combination of enhanced sociableness, reduced fatigue, and exercise-induced exhaustion after a workout, followed by microstimulation, has important implications for professional sporters and nonprofessionals who try to get the best result after a workout. Future studies using a double-blind approach including different types of exercises, different durations of programs, and both sexes can shed more light on the full potential of microstimulation after a workout on mood state and exercise-induced exhaustion. About one third of patients with epilepsy have seizures refractory to the medical treatment. Electrical stimulation mapping is the gold standard for the identification of “eloquent” areas prior to resection of epileptogenic tissue. However, it is time-consuming and may cause undesired side effects. Broadband gamma activity recorded with extraoperative electrocorticography during cognitive tasks may be an alternative to ESM but until now has not proven of definitive clinical value. Considering their role in cognition, the alpha and beta bands could further (...) improve the identification of eloquent cortex. We compared gamma, alpha and beta activity, and their combinations for the identification of eloquent cortical areas defined by ESM. Ten patients with intractable focal epilepsy participated in a delayed-match-to-sample task, where syllable sounds were compared to visually presented letters. We used a generalized linear model approach to find the optimal weighting of each band for predicting ESM-defined categories and estimated the diagnostic ability by calculating the area under the receiver operating characteristic curve. Gamma activity increased more in eloquent than in non-eloquent areas, whereas alpha and beta power decreased more in eloquent areas. Diagnostic ability of each band was close to 0.7 for all bands but depended on multiple factors including the time period of the cognitive task, the location of the electrodes and the patient’s degree of attention to the stimulus. We show that diagnostic ability can be increased by 3–5% by combining gamma and alpha and by 7.5–11% when gamma and beta were combined. We then show how ECoG power modulation from cognitive testing can be used to map the probability of eloquence in individual patients and how this probability map can be used in clinical settings to optimize ESM planning. We conclude that the combination of gamma and beta power modulation during cognitive testing can contribute to the identification of eloquent areas prior to ESM in patients with refractory focal epilepsy. Jackson says that the form of physicalism that I recommend, with certain emendations he believes are necessary, turns out to be none other than the “Australian” type-type identity theory of J.J.C. Smart and others. About this, too, I have no serious disagreement, although Jackson’s claim appears to depend, at least in part, on a certain chosen reading of the texts involved. In fact, one point of similarity may be worth noting. As I take it, one special feature of the “Australian” (...) type identity theory is the claim that mental-physical identities are contingent, not necessary. Suppose M has a single physical realizer, P. On my account, the statement that M = P is not metaphysically or logically necessary. It is contingent since the statement that P realizes M is contingent. This is so because for P to realize M is for it to fulfill the causal specification that defines M, and what causal relations P actually enters into is a question that depends on the laws prevailing in a given world. Thus, P may be a realizer of M in this world but perhaps not in another in which different laws hold; in some worlds M may have no realizers at all. None of this is in violation of the idea that identities are necessary. For, according to me, if M has realizers, M is a functional property defined by a causal specification. This means that “M” is not a rigid designator, though it is, as we might call it, “nomologically rigid” or “semi-rigid”. (shrink)

In his Introduction to (...) Logic 1 Irving Copi raises a problem with two of these definitions which he believes forces him to limit the range of propositions which can be used in the square of opposition. Since contrary propositions can both be false, but the falsity of a necessarily true proposition is not possible, a necessarily true proposition has no contrary. Therefore, only contingent propositions can be contraries. Subcontrary propositions can both be true, but the truth of a necessarily false proposition is not possible.2 This problem is rarely raised in other logic texts, but, it is interesting to note, it is mentioned by a number of neoscholastic philosophers, for example, P. Nicholas Russo,3 Rev. H. Grenier,4 1 Irving M. Copi, Introduction to Logic (New York: Macmillan, 1986), pp. 178-179. Copi first introduced this problem in the fifth edition of his work on the basis of an article by David H. Sanford entitled " Contraries and Subcontraries,'' Nous 2 (1968) : 95-96. It should be mentioned, however, that the position of the philosophers cited above is not entirely the same as Copi's. Copi reasons that these definitions imply that necessary propositions have no contrary or subcontrary. The other philosophers believe that necessary propositions can be contraries and subcontraries. Thus, in the case of necessary propositions contrary and subcontrary propositions are, instead, defined merely in terms of their quantity and quality, not by whether or not they can both be false or can both be true. AimWe aimed to investigate whether the 12-item Multidimensional Scale of Perceived Social Support (MSPSS) constitutes a valid and reliable measure of social support for the general adult Australian population.MethodsData were from Australia’s National Survey of Adult Oral Health 2004–2006 and included 3899 participants aged 18 years old and over. The psychometric properties were evaluated with Bayesian confirmatory factor analysis. One-, two-, and three-factor (Significant Other, Family and Friends) structures were tested. Model fit was assessed with the posterior predictivep-value (PPPχ2), Bayesian (...) root mean square error of approximation (BRMSEA), and Bayesian comparative fit index (BCFI). Dimensionality was tested by comparing competing factorial structures with the Bayes factor (BF). Reliability was evaluated with the Bayesian ΩH. Convergent validity was investigated with the Perceived Stress Scale (PSS) and discriminant validity with the Perceived Dental Control scale (PDC-3).ResultsThe theoretical three-factor model (Significant Other, Family, and Friends) provided a good fit to the data [PPPχ2< 0.001, BRMSEA = 0.089-95% credible interval (CrI) (0.088, 0.089); BCFI = 0.963-95% CrI (0.963, 0.964)]. The BF provided decisive support for the three-factor structure in relation to the other structures. The SO [BΩH= 0.95 - 95% CrI (0.90, 0.99)], FA (BΩH= 0.92 - 95% CrI (0.87, 0.97), and FR (BΩH= 0.92 - 95% CrI (0.88, 0.97)] subscales displayed excellent reliability. Jackson says that the form of physicalism that I recommend, with certain emendations he believes are necessary, turns out to be none other than the “Australian” type-type identity theory of J.J.C. Smart and others. About this, too, I have no serious disagreement, although Jackson’s claim appears to depend, at least in part, on a certain chosen reading of the texts involved. In fact, one point of similarity may be worth noting. As I take it, one special feature of the “Australian” (...) type identity theory is the claim that mental-physical identities are contingent, not necessary. Suppose M has a single physical realizer, P. On my account, the statement that M = P is not metaphysically or logically necessary. It is contingent since the statement that P realizes M is contingent. This is so because for P to realize M is for it to fulfill the causal specification that defines M, and what causal relations P actually enters into is a question that depends on the laws prevailing in a given world. Thus, P may be a realizer of M in this world but perhaps not in another in which different laws hold; in some worlds M may have no realizers at all. None of this is in violation of the idea that identities are necessary. For, according to me, if M has realizers, M is a functional property defined by a causal specification. This means that “M” is not a rigid designator, though it is, as we might call it, “nomologically rigid” or “semi-rigid”. (shrink)

This paper addresses an issue with the sign ‘⊢’ in Frege’s mature version of Begriffsschrift, i.e., the version in ‘Function and Concept’ and Grundgesetze. The sign is a performative for asserting in that writing down ‘⊢p’ is equivalent to asserting that p. Frege further says that writing ‘ p’ is also equivalent to identifying the reference of ‘p’ with the truth-value True. It looks as if he holds that asserting that p consists in identifying the True with the reference of (...) ‘p’. Frege’s commitment to it, however, seems to encounter a number of tensions. This paper aims to show that these tensions can be avoided by endorsing a non-assertive conception of identification under which making an identification is not making an identity assertion. The suggested reading leads to an entirely di erent understanding of the compositionality of the sign ‘⊢’ as well as Frege’s conception of assertion and judgment. (shrink)

Little is known about how researchers view ethically salient aspects of human studies. As part of a National Institutes of Mental Health-funded study, the authors performed a confidential written survey to assess the attitudes, views, and experiences of researchers with institutional review board approved protocols at the University of New Mexico. A total of 363 researchers (57% response rate) participated. Investigators overall held favorable views of general ethical aspects of research and ethics-based safeguards, and they identified a positive role of (...) ethics training. Investigators with more experience encountering ethical problems (p < .001), more ethics training (p = .001), and a PhD or MD/PhD (p = .003) held more favorable general ethical perspectives. Women investigators (p < .03), nonphysician investigators (p < .001), those whose training had been helpful in resolving ethical dilemmas (p = .006), and those for whom spirituality is important (p = .008) more strongly endorsed ethical safeguards. Investigators perceive the scientific and ethical aspects of their work as valuable and linked, and they affirm the role of safeguards in human studies. Formal ethics preparation and training initiatives were also viewed positively by investigators. (shrink)
